Estring is a long term way to deliver vaginal oestrogen. It is a thin silicone ring, which is 5.5cm in size that is inserted into the vagina every 3 months. The Estring releases slightly lower amounts of oestrogen every 24 hours compared to pessaries, but it's great that it's a "fit and forget" treatment and you can even have sex with it in. Vaginal oestrogen is very safe, with studies showing it is only absorbed into the body for a very short time at the start of use, in fact, an entire year's supply of vaginal oestrogen is equivalent to just 1 oral oestrogen HRT tablet. The leaflet contained in the packet of vaginal oestrogen is the same leaflet used in oral and patch oestrogen and does not fully apply to vaginal use. Large studies have shown there is no increased risk of cancer when using vaginal oestrogen. Women who have had breast cancer may also use vaginal oestrogen, but only after a discussion with their oncology specialist.

My experience using HRT has been a journey. I have tried different regimes. I first starting on tablets, but due to my migraines I was put on patches. I went on the combined Evorel Conti patch but that made me have a withdrawal bleed every day for 6 months. I felt awful and weak. I then tried Evorel Sequi. This combined patch stopped the bleeding, however there wasn't any change in my symptoms. I was on 50 micrograms with these combined patches Two years in I started to experience vaginal atrophy. I tried Vagifem but it just caused vaginal burning, then ovestin, but this didn't work for me either and I found it to be very messy. After extensive research, I came across Estring, which a vaginal ring, it releases a steady amount of estrogen daily into the vagina keeping it hydrated. This was a life saver. Due to my symptoms not improving, the consultant put me on a higher dose patch due to my age. As the combined patches only go up to 100 micrograms, I had to be given an Estradiol only patches with a progestogen, again as i have a womb. I used Utrogestan but that caused bleeding. I stuck it out for 6 months then gave up. It also made me very dizzy. I'm now now using the Marina coil in conjunction with the transdermal estradoil patch as I have a womb. My current dose is 137.5 micrograms which is Estradot 100 and Estradot 37.5. I use two patches twice a week

I did gain weight, but I don't think that's due to the HRT. The bleeding was due to changing from the mirena coil to utrogestan which didn't agree with me - it cause reflux, mood changes and nausea. I now have a new mirena coil and am very happy.

My experience is mainly post menopausal and triggered by severe Genito symptoms of the menopause. My life was so debilitated by this that I had to go on a HRT regime to get some Iife back at the age of 64. It has helped through a period of trial and error. Tried Vagifem and Intrarosa both caused irritation. I needed to add transdermal and Estring to get relief.The NHS has offered little support . My prescription is on the NHS but no one reviews it at all . I’ve had to wait one year for a phone call from Menopause clinic to be triaged on the phone. I had so much irritation I was convinced ingredient in HRT vaginal creams were causing it. I was assessed by dermatologist and an allergy panel. I was not allergic to the ingredients . Some of the ingredients are irritating to the skin however and should not be in these gels. The Estring solved the problem for me along with a little blissel or ovestin externally. I suspect the oestrogen may be contributing to an increase in incontinence and prolapse as I am hyper mobile and have lax ligaments. A women’s health physio states I now have a hypermobile urethra . So whilst HRT has stopped the symptoms of incessant burning it has caused other symptoms to be enhanced . It’s a balancing act to get it right and it is not easy to do.

It has been a long process to settle on a regime via my private consultant. For some time I was taking higher than recommended doses as I am one of many who do not absorb well. Tried various brands and dosages but no real difference so I have now reduced my dosages to more normal levels with no effect to how I am. It’s important to keep tweaking dosages and different drugs to get the right combination.
